Policies and Procedures for Claims

To whom it may concern:

The infusion and procedure services provided at The RSD / CRPS Treatment Center and Research Institute are performed in a State licensed and approved outpatient surgery center.  The surgery center has met all of the criteria for licensing and the governing bodies requirements.

Infusion and procedure services performed at the center are billed at a rate inclusive of the facility fee and of the professional fee.  These fees and procedure codes are noted on the following website:

We will gladly furnish these codes to you if requested.  The center performs these procedures under location code 24 as billed on a HCFA 1500 form. 

The RSD / CRPS Treatment Center and Research Institute does not participate with any insurance company nor does it accept assignment of benefits.  We will be willing to provide documentation of the services provided to aid our patients in filing their claims with their insurance carrier.  If any further information is required please feel free to request such information at any time.
